Our goal is to be the provider of last resort for the children and teens of Yolo County. In order to pair children with a medical home please utilize these resources:

 

Maternal, Child, Adolescent Health (MCAH)

The mission of the Yolo County Maternal, Child, and Adolescent Health (MCAH) Program is to assure that pregnant women and their children are able to obtain quality maternal and child health services.

Child Health & Disability Prevention (CHDP)

The CHDP Program is a preventive program for infants, children, and teens that provides complete health exams and immunizations for Medi-Cal eligible children (ages 0 to 21) and moderate income children (ages 0 to 19) at no cost. Services in Yolo County are provided by enrolled physicians and community clinics. CHDP provides care coordination to assist families with medical appointment scheduling, transportation, and access to diagnostic and treatment services.
