What will clients be able to do at the “Wellness Alternatives for Adults” Program?

  • Establish a service plan that identifies goals appropriate to the individual’s needs and abilities.
  • Receive services that promote wellness, recovery, resilience and independent living.
  • Find and maintain affordable housing.
  • Access community resources.
  • Receive assistance in job readiness.
  • Participate in self-help services with peers.
  • Access help with securing appropriate benefits.
  • Develop a network of family and friends.
  • Integrate substance abuse and mental health services in a single service plan.

 

Pathways to Independence Transition Age Youth Center

  • Establish a service plan agreement that identifies goals appropriate to the individual’s needs and abilities.
  • Receive services that promote wellness, recovery, resilience and independent living.
  • Find and maintain affordable housing.
  • Access community resources.
  • Receive assistance in job readiness.
  • Participate in self-help services with peers.
  • Access integrated educational services and support to complete high school diploma.
  • Receive support and encouragement in the pursuit of higher education.
  • Receive supportive services to individuals emancipating from Foster Care or from Juvenile Hall.
  • Access help with securing appropriate benefits.
  • Develop a network of family and friends.
  • Access substance abuse and mental health services in a single service plan.

What will clients gain from the Greater Capay Valley Children’s Pilot Program?

  • Access to Family Partners and Community.
  • Members that will provide peer and family support, community engagement, outreach, family education, information and referral services.
  • Increased opportunities for their level of participation and involvement in all aspects of the public mental health system.
  • Opportunities for mental health services for rural residents.
  • Increased community supports for children and youth diagnosed with serious emotional disorders and their families.
  • Mentoring opportunities for children and youth.
  • Increased awareness by children, youth and families regarding mental health diagnosis, assessment, medications, services and supports planning, treatment modalities and other information related to mental health services.

 

Older Adult Outreach and Assessment Program

Sometimes older people develop health, social, or emotional problems that make day-to-day life difficult. The Older Adult Outreach and Assessment Program is a resource for these persons. Program Staff may assist seniors and their families by helping to identify areas of concern, offering emotional support and arranging appropriate services. The primary goal of the Older Adult and Assessment Program is to assist older adults in making their own choices.
