Submitting Documents, Applications, and Forms
Applications and all required items may be mailed, emailed, or placed in the drop box at the Yolo County Health Department. The drop box located across from the stairs labeled YEMSA drop box and is the preferred method.

If you would like to meet with someone to drop off your paperwork, please make an appointment by calling 530-666-8665.  Using the drop boxx is preferred!


Application Fees

  • Late Application Fee $13.00 Applications are considered late if turned in between 0-30 days of expiration date.
  • Replacment/Duplicate Card $13.00
  • EMR Application $15.00
  • EMT Initial Certification $90.00 (includes State fee)
  • EMT Re-Certification $52.00 (includes State fee)
  • Paramedic Accreditation $53.00
  • Paramedic Lapsed Accreditation $53.00
  • Paramedic Reaccreditiation - None               
  • No cash or personal checks accepted.  Please make cashers check or money order payable to: Yolo County EMS (all fees are non-refundable)
